7/15/2023 EnchantaVaughn Approved Behavior: The family showed positive interactions during todayâs session. They interacted well with one another and spoke to each other calmly and effectively. The family laughed and joked with one another and needed little redirection on their dialogue or interactions. Client was thankful that her counselor provides one on one help when she needs it and expresses happy thoughts about being in session. Client enjoys receiving compliments and rewards for her hard work. Client remains on task and is observed following directions promptly. Behavior: Client is reminded to think before she acts and mindfully consider all solutions before reacting to conflict or problem. Client responds well to feedback. Client was showing a positive attitude in all areas today. Client was very attentive during interventions interacting with QMHP in a calm manner and did not raise her voice or display negative tone. Behavior: Client openly discussed her feelings about her current state and how she feels. Client appeared to be in a good mood as evidence by her participating in family activity without conflict. Client remained on task and in cooperation with home rules. Intervention: QMHP used the basic listening sequence to clearly draw out the point of view that the client was feeling and get beyond her aggressive language and behaviors. QMHP implemented individualized behavior management techniques with client. QMHP processed with client about psychoeducational activity while assisting with directions. QMHP explained the importance of the activity. QMHP provided feedback to client in regard to questions asked. Intervention: QMHP explored clientâs responses and provided appropriate feedback. QMHP complimented client for utilizing acquired skills, paying attention, and focusing on the importance of the assignment. QMHP supported client displaying positive social interaction with authority. Intervention: QMHP modeled proper positive behavioral expectations for the client to encourage additional layers of support. QMHP commended client on her efforts to maintain her objectives and coping with stressors without needing additional redirection or prompts. Response: Client discussed mental illness and its causes as a family. Client said she was aware of her expectations and what she is expected to do each day. Client demonstrated moderate progress during the session. Client was able to communicate with the QMHP as information was shared. Client communicated that she felt safe and comfortable talking with the QMHP. Client was attentive and open with counselor about her feelings and how to identify them. Client shared that she believes she will be able to master her feelings soon and control her anxiety. Response: Client showed good understanding of what it means to be respectful, encouraging, mindful, and fair to others. Client is observed correcting her mistakes and making good choices. Response: Client was able to utilize coping skills to eliminate stressors and unwanted feelings. As the client developed understanding of home expectations, she reported increased satisfaction and function. Client shared that she enjoyed the session she had with counselor and canât wait for the next session.
